San Carlos activates online vax registration

SAN CARLOS CITY, Pangasinan – The online vaccine registration developed by the local government unit here has already been activated to determine the vaccine acceptability among residents.
 
This, as senior citizens here are next in line for the vaccination program wherein the city government is targeting to inoculate at least 18,000 elderly based on the master list of the city health office, city information officer Jhuliano Nazareno said.
 
In an interview on Thursday, Nazareno said applicants to register for the coronavirus vaccination program can access the link of the Google form at the Facebook page of San Carlos City Information Office, or scan the quick response (QR) code and log in with a Gmail account.
 
He said they should provide information to various questions and attach a necessary picture or documents that would prove their inclusion to the priority group. 
 
Nazareno said that upon submission, provided information will be sent directly to their master list and will be forwarded to the Department of Information and Communications Technology and Department of Health (DOH).
 
Kapag ito na ay aprubado ng DOH, magpapadala ng karampatang bilang ng bakuna na may listahan ng tatanggap nito mula sa prioritized master list (When approved by the DOH, they will send appropriate vaccine allocation with the list of individuals who will receive the vaccine from the master list),” he added.
 
Nazareno said applicants will receive a text message with the time, day, and location where they are scheduled for inoculation.

However, due to the strict implementation of health protocols in the city, the scheduled opening of mall-based registration was postponed until the end of May, or the first week of June this year. (PNA)
