Human error causes QC vax registration mix-up

MANILA – The Quezon City government on Tuesday apologized for reported discrepancies in the recording of vaccine administration in one vaccination site and assured its constituents that they are working to reconcile the disparity.

QC Task Force Vax to Normal co-chair Joseph Juico said in an interview with the Philippine News Agency that the incident was an isolated case and was caused by human error.

Juico said they received a complaint about errors in the booking system eZconsult.

“During the investigation, what happened was a patient went to Araneta Coliseum, the patient ID number was encoded, and what was encoded was the patient ID number of the father, because all it takes is one number for you to type in wrong and it will generate a different information,” Juico explained.

In a statement, the city government said it was the first time it happened and they are looking into the incident.

“We sincerely apologize for any confusion this may have caused some of our vaccine patients. Rest assured that the local government and eZConsult will address the matter to prevent its reoccurrence,” the statement further read.

Mayor Joy Belmonte assured they are working to reconcile the discrepancies and advised the public to always refer to their vaccination card for the correct brand of vaccine and the date of the second dose.

While the incident was an isolated case and attributed to an encoding error, Belmonte said it should not happen again.

“What happened was an encoding error, but having said that, it does not take away the accountability from the city government and that this should not even be happening and therefore the city government is grateful for the feedback that the citizens give to us or inform us about,” Belmonte said.

Belmonte also called on residents to report whatever error or complaints they have so that the city government will be able to address those and improve the system.
